Pharmaceutical services contractor survey

Overview

The Pharmaceutical Needs Assessment (PNA) sets out the pharmaceutical services which are provided in Southwark, together with when and where these are available to the population.

 To ensure that pharmacy-based public health services are provided which meet the needs of local people, it is essential to regularly review the quality, accessibility and breadth of provision. It is also important to encourage local primary care providers to promote public health to local communities.

 For this reason, we would be grateful if you could complete this survey. Your data will be used to support the PNA, which Southwark Council is statutorily required to prepare.

 All data collected will be processed in accordance with the Data Protection Act 1998. No personally identifiable information will be shown in the report.
