Overview

This survey has been developed jointly between NHS South East London Clinical Commissioning Group, Improving Health Limited and Quay Health Solutions. 

The way in which you may have accessed healthcare over the last few months has changed as a result of the COVID 19 pandemic. The pandemic will continue to have an impact on the way in which a number of services are accessed. This short survey is to help Southwark GPs better understand your experience so far, so that we can plan better for the future – we want to know how to improve so do be honest about difficulties. 

We apologise that this survey can only be completed electronically. If you know someone who would like to complete this survey who does not have access to the internet then that person could speak on the telephone to someone else who completes it on their behalf. Alternatively the survey can be printed, completed and scanned to SOUCCG.southwark-ccg@nhs.net

Please remember, your GP surgery continues to be open as per normal working hours. Doctors, nurses and other staff ready to help you if needed. The main thing that has changed is that all appointment requests are first discussed by telephone or online consultations. So please do not delay seeking help if you are worried about your health. Call your surgery or visit their website to make contact.
